Claim your credit when you file your Arizona income tax return.
Include: Arizona Form 321 (QCO) and the summary, Arizona Form 301.

Step 3

Credits are nonrefundable; any unused amount carries forward up to 5 years.

*If you receive a state tax credit, your federal charitable deduction must be reduced by the credit amount. Only amounts above the AZ QCO limit may remain deductible federally (subject to usual rules).
